I, (printed name): the parent/guardian of the individual, a minor (the “Athlete”), do hereby permit the Athlete to participate in a series of ICAN Player Development training sessions and certify that the Athlete’s physical condition is sufficient for full participation. I understand that the Athlete’s participation in ICAN Player Development involves an element of risk and a possible danger of accidents. I also understand that the Athlete's participation at ICAN Player Development may include activities involving athletes of different ages participating together. I assume all risks and agree to hold harmless ICAN Player Development LLC, and all owners, directors, coaches, staff members, volunteers, heirs, and the property owners from all claims resulting from accidents, bodily injuries and illnesses that may arise from participation in training events and activities involving ICAN Player Development. Knowing all these risks, I hereby assume these risks and I hereby release and discharge ICAN Player Development from any and all liability resulting from the Athlete’s participation in any aspect of ICAN Player Development.
I understand it is my responsibility to inform ICAN Player Development staff of any medical conditions or any other special needs the Athlete might have and will notify the appropriate individuals of any health issues that might in any way affect the Athlete’s active or passive participation in a ICAN Player Development training session. I hereby assume responsibility for any and all costs associated with treatment of the Athlete for any injury or health issue that arises during the Athlete’s participation in a ICAN Player Development training session. I understand that ICAN Player Development is not responsible for the supervision or safety of the Athlete if the Athlete leaves the campus for any reason during the course.
LOCATION AGREEMENT:
By registering for a camp, you acknowledge and agree that all sales are final. While we make every effort to hold camps at their advertised locations, facility cancellations or other circumstances beyond our control may occasionally require us to move a camp to an alternate location.
If a location change becomes necessary, we will make reasonable efforts to relocate the camp within a 7-mile radius of the original location. Due to occasional school district and facility cancellations, families should be prepared for the possibility of a location adjustment.
It is the responsibility of each participant's family to arrange transportation to the camp location, including any alternate location within this radius. Because the camp will still be provided, a relocation within the 7-mile radius does not qualify for a refund, credit, or cancellation.
We will communicate any location changes as promptly as possible and appreciate your understanding and flexibility.
I understand that all sales are final and there are no refunds available.
I the parent/guardian of the individual, also hereby grant permission to ICAN Player Development to use photographs and/or video of the participating athlete taken during training sessions in publications, news releases, online, and in other communications related to the mission of ICAN Player Development.
